Fees & Payment

For all billing and accounts questions please contact our Financial Director, Cathy Welker, at (320) 405-8261. You may also fax her at (320) 373-5056. Insurance will reimburse in most cases. In situations where it does not, or you choose not to use an insurance plan, fees are private pay. A sliding scale, based on your situation, is available if needed. We do our best to work with you and provide a service that is affordable and realistic financially. Also, many churches are willing to contribute all or a portion of the fees.

Insurance

At Crossroads we accept a variety of different types of commercial, business, state, and individual health insurance plans. Please call your insurance company if you are unsure about your personal health insurance plan including mental health benefits. You may also contact our business office with insurance related questions.

What do I need to bring to my first appointment?

Please bring your insurance card, or any other information regarding your payment plans. Many of the forms in our initial paperwork may be filled out ahead of time. Please click on the appropriate age range of the client; then you may fill out, email or print and bring the documents to your initial session.
